To keep the hand steady while applying a well placed little finger on the cheek will work wonders to combat jittery hands.

While standing in front of a well lit mirror tilt the head slightly back and half close the eyes (just enough so you can still see what is happening) this will allow close application to the eyelash line.

There are small differences in eyeliner application that should be taken into account depending on the setting of the eyes, Deep set eyes, Small eyes, Drooping eyes, and Deep set eyes to name a few.
